You need a conquistador and land on them or keep sailing nearby with an explorer. It can take a long time with an explorer but if you sail in and out of the map tile or in a path through many map tiles with islands eventually you will clear the terra incognita.

In order to explore past terra incognita you need an army or navy that’s lead by a conquistador or explorer, respectively. You can hire these leaders with the regular generals/admirals in the Military tab, and you can assign them to an army/navy when you select the unit in friendly territory.
